We are pleased to announce that the online pre-admissions form is now available on our websites.
Patients can complete and submit their pre-admission information via the website and upload their supporting documents (i.e. Copy of their Medical Aid card and identification document). The completed form and supporting documents will be submitted to our admission team at the hospital, who will capture the required information before the patient’s admission date providing patients with a faster and more efficient admission process.
The steps for the patient to follow are detailed below as well as provided in the infographic on the second page of this document:
Step 1 – The patient would simply visit the website of the Busamed hospital that they will be admitted into
Step 2 – Navigate to the Your Visit Page Tab and click on the link
Step 3 – Scroll down to Preparing for Your Stay and click on the link – Pre-Admissions
Step 4 – The patient would simply complete the online form and upload a copy of their identity document (ID, Drivers license or passport) and Medical Aid card (if applicable) and select submit
Step 5 – Our team will capture the patients information before the patients date of admission
Step 6 – Patient arrives at the hospital on the day of admission with their original identification document and medical aid card (if applicable)
