We encourage all patients to take advantage of our pre-admission service before their admission date with us. Our friendly staff are available to help you complete the pre-admission forms as well as answer any questions that you may have.
Completing the pre-admission process before your admission date will assist in making your admission on the day as streamlined and hassle-free as possible.
For your convenience, there are two methods by which you can submit your pre-admission documentation:
We encourage you to visit the pre-admission nurse or clerk based at the hospital. They will be able to guide you through the process and assist you with the completion of the documentation.
Should you wish to complete and submit your documents at your own convenience, you may complete the on the website and submit your supporting documents. We kindly request that this is sent 72 hours before your admission to ensure that there is enough time to capture your details and to contact you should we require any other information.
Please attach copies of your identity document (RSA ID, Drivers License or Passport), Medical Aid Card and your doctor’s referral letter for admission.
